Fit & Well: “National Case Management Week”

TWIN FALLS, Idaho (KMVT/KSVT) —Navigating healthcare needs can be complicated, but hospital case managers help patients have support for a smooth recovery.

“National Case Management Week” begins on Monday, and that’s the focus of this week’s Fit and Well Idaho report.

In a hospital, case managers look at a patient’s needs and connect them to the right services and resources. They also help develop discharge plans for patients leaving the hospital to make sure their needs are met at home. This includes things like arranging medical equipment, transportation for follow-up visits or home health care.

Brad Hoyer, Saint Luke’s Case Management Supervisor, talks about what it takes to qualify for a position like this, “Usually we like to see the nurses and the social workers have about five years of experience working at the bedside or with patients in general, and then we train here at St. Luke’s, we do an orientation process.”

“National Case Management Week”, which is the second full week of October, looks to recognize the work they do to help people receive the proper care needed.
